APPLICATION FORM

Midway Fire Academy will be offering a First Responder Course consisting of a total of 40 hours, including AHA CPR for the Healthcare Provider. First Responder certification is a pre-requisite for becoming a certified firefighter in the State of Florida. The course will be held at Midway Fire Training Facility located at 1801 Abercrombie Rd, Gulf Breeze, FL 32563.  Attendance is mandatory.

The First Responder is a designated level of emergency medical care provider as outlined by the National EMS Education and Practice Blueprint. It is the goal of the First Responder Course to provide students with the core knowledge, skills and attitudes to function in the capacity of a first responder. The First Responder uses a limited amount of equipment to perform initial assessment and intervention and is trained to assist other EMS providers. The course will cover all objectives set forth by the US Department of Transportation, National Highway Traffic Safety Administration, as well as the State of Florida.

Students will need to mail, or drop off, a registration form along with the tuition fee, to Midway Fire District Administration during the open enrollment dates. Books will be supplied on a zero-cost rental basis for the duration of this course. The Course fee will be $100.00, and includes CPR fees.
